Nominations open October 15, 2025. The deadline to submit a nomination is December 15, 2025 at 11:59 pm.

Before preparing a nomination, review the award criteria nomination requirements on the Continuing Professional Development awards webpage.
Once started, it is not possible to save your progress and return to the form later.
You can preview the nomination form here.
To avoid losing work in the event of technical issues, we recommend that you draft your responses in Word and paste them into the online form.
