External Teaching Awards Nomination Form

For external awards, candidates are first adjudicated at the faculty level to select the official nominee that will be put forward on behalf Temerty Medicine.

• The first stage of the adjudication requires a simpler version of the official nomination package. If chosen as the official Temerty nominee, the Office of the Vice Dean, Medical Education will support the nominee to develop their full nomination package.
• Before preparing a nomination, review the award criteria nomination requirements using the awards database.
• You can preview the nomination form here.
• Once started, it is not possible to save your progress and return to the form later.
• To avoid losing work in the event of technical issues, we recommend that you draft your responses in Word and paste them into the online form.
