External Teaching Awards Nomination Form

For these awards, nominations are first adjudicated at the Faculty level to select the candidate that will move forward as the official Temerty nominee:

The first stage of the adjudication requires a simplified version of the official nomination package. You can preview the nomination form here.
If chosen as the official Temerty nominee, the Office of the Vice Dean, Medical Education will support the nominee to develop their full nomination package by the external deadline.
Please direct any questions to medicine.awards@utoronto.ca rather than to the organization offering the award.
A nominee can be put forward for only one award per year.

Once started, it is not possible to save your progress and return to the form later.
To avoid losing work in the event of technical issues, we recommend that you draft your responses in Word and paste them into the online form.

Before preparing a nomination, review the award criteria and nomination requirements using the awards database.

All other external awards are part of the spring cycle which will open for nominations in mid-March 2026.
